Intensive OCD Therapy Palo Alto

For some people with OCD, weekly therapy isn’t enough. When intrusive thoughts and compulsions are consuming hours of the day, interfering with school or work, and making everyday life feel impossible to manage, a more intensive level of support can make the difference.

Intensive OCD therapy may be right for you if:

  • OCD rituals and compulsions are taking up hours of every day
  • Intrusive thoughts are causing severe distress and anxiety
  • OCD is interfering with school, work, or social life
  • Avoidance of places, people, or situations is growing
  • Weekly therapy has not produced enough relief
  • Symptoms have been escalating despite previous treatment attempts

Our Intensive OCD Therapy program offered both in person or online structured, evidence-based treatment for children, teens, and adults — delivering faster, more significant progress without the disruption of inpatient care.

What Is Intensive OCD Therapy

Intensive OCD Therapy is a structured 8 to 12 week program that offers a significant step up from once-weekly therapy without requiring inpatient treatment. Rather than a single session per week, the intensive program incorporates multiple appointments each week — including individual therapy sessions, exposure coaching, and for families with children or teens, dedicated parent guidance sessions.

The foundation of our Intensive OCD Therapy program is Exposure and Response Prevention — ERP — the gold standard treatment for OCD and the most thoroughly researched approach available.

ERP works by helping individuals gradually face the thoughts, situations, and triggers that OCD has made frightening, while resisting the urge to perform compulsions or seek reassurance. Over time, the brain learns that the feared outcome does not occur, and the anxiety associated with the trigger begins to lose its power.

At the start of the program, your therapist works with you to build a personalized hierarchy of feared situations — from those that feel manageable to those that feel most distressing.

Exposures begin at a pace that is challenging but not overwhelming, and progress as confidence and tolerance build. For someone with contamination fears, this might begin with touching a commonly used surface without immediately washing. For someone with intrusive thoughts about harming a loved one, it might involve being near a sharp object without seeking reassurance. Every exposure is carefully planned and fully supported.

Exposure and Response Prevention

ERP is the most effective treatment available for OCD, with decades of research behind it. Rather than talking about OCD, ERP involves actively facing it — in a structured, supported, and carefully paced way. Your therapist builds a personalized exposure hierarchy with you, starting with manageable challenges and gradually working toward the situations and thoughts that feel most distressing. The result is a brain that no longer responds to OCD triggers with the same intensity of fear.

More Sessions, Faster Progress

The intensive format — multiple appointments per week over 8 to 12 weeks — is what sets this program apart from standard weekly therapy. OCD is a condition that responds strongly to consistency and repetition. When treatment happens more frequently, there is less time between sessions for avoidance to take hold and more opportunity to build momentum. Many clients who have struggled for years in weekly therapy experience meaningful relief within the first few weeks of the intensive program.

Family Involvement & Real-World Coaching

For children and teens, family involvement is a critical part of effective OCD treatment. Parents receive dedicated coaching sessions to understand how OCD works, recognize the accommodations that unintentionally maintain it, and learn how to support their child’s progress at home. Where appropriate, exposure coaching can also take place in real-world settings — at home, at school, or in the community — ensuring that skills are practiced exactly where OCD shows up most.

Frequently Asked Questions About OCD

Intensive OCD therapy is the right fit when OCD symptoms are significantly disrupting daily life often consuming hours each day, interfering with school or work, or causing severe distress that weekly therapy hasn’t been able to address. It’s also a strong option for anyone who has tried therapy before without meaningful relief, particularly if that therapy didn’t include ERP.

This is one of the most common reasons people come to our intensive program. Effective OCD treatment requires ERP, not just talking about OCD, but actively practicing facing it. Many people have experienced therapy that focused on understanding their thoughts rather than changing their relationship to them. If previous therapy didn’t include structured ERP, it’s very much worth trying again with a specialist.

Never. ERP at Palo Alto Therapy is always collaborative. Your therapist builds the exposure hierarchy with you and moves at a pace that is challenging but never overwhelming. The goal is to build genuine confidence and tolerance — not to push anyone past their limits. You are in control of the process every step of the way.

The intensive program typically runs 8 to 12 weeks, though the exact length depends on the individual’s needs and goals. Many clients transition to standard weekly therapy after completing the intensive phase to consolidate their progress and maintain gains over the longer term.

Some components of the program — such as parent guidance sessions and certain therapy appointments can be conducted via secure video. Exposure therapy, however, is most effective in person, particularly when it involves real-world triggers. We work with each client to find the right balance of in-person and remote sessions based on their specific needs and circumstances.

The program is available for children, teens, and adults. The structure and specific techniques are tailored to each person’s age and circumstances for adults, the program focuses entirely on individual therapy and real-world exposure work, while for younger clients it incorporates family involvement and school-based coaching where needed.
